Our highly trained & experienced Support Coordinators have deep networks in local community, are fully immersed in the NDIS ins and-outs, and keep ahead of the details and changes. We stand side by side with NDIS Participants through their NDIS journey.
We provide Support Coordination services across North Queensland from Mackay to the Torres Strait Islands.
Our team has rich networks and localised knowledge of culture and services. Through experience, we have streamlined experience and expertise in specific support categories, like Hospital Discharges,
Psychosocial Recovery Coaching, Aboriginal and Torres Strait Islander Family Support, and Deaf Community coordination.
Our team members come from customer service backgrounds, they are from diverse backgrounds and cultures, and they are local to their communities. As a result, we are relatable to our clients; we are highly trained and experienced in NDIS processes, and we have an incredible work ethic - we are here, we have time, we are on your side.
Our Mainstream Support
Coordinators have expert knowledge of service offerings – including mainstream, community, and informal supports – and have strong provider networks in the participant’s local area. Within this team, we have technical advisors in culture, language, sensory supports, education, housing, and child safety
Our Hospital to Home Support
Coordinators are highly experienced in hospital/respite transitions to home/country. Lead by our First Nations Executive Manager, Josh, this team works directly with the local hospitals, aged care facilities and the NDIA to move people out of inappropriate accommodation, into a home with supports. This team specialises in Specialist Disability Accommodation, Assistive Technology, and Supported Living Arrangements.
Complex Support Needs Pathway
Provides Specialised Support Coordination for participants living with a disability who have many different challenges impacting on their lives such as mental health issues, incarceration or homelessness, and need a higher level of specialised supports in their plan.
