We are experts in Support Coordination and Plan Management services, with a prominent focus on remote regions. We understand the importance of delivering a culturally safe and competent service to ensure fair and equitable access to all.
We understand the importance of Aboriginal and Torres Strait Islander cultural practices, traditions, and protocols within the various communities. Xtremecare is committed to ensuring that our service is tailored to each individual and community’s needs.
We aim to build up the capacity of participants and communities to empower them to deliver NDIS services in their community, which will create long term sustainability in the future.
We provide our Plan Management service nationally, our Support Coordination service is delivered only in the Western Cape Region, Northern Peninsula Region, and Torres Strait Regions.

We are originally from Cairns. My husband is Yidinji, my daughters are Kuku-Yalanji and Irukandji as well as Torres Strait Islanders. I am a proud Ugar and Erub Torres Strait Islander woman.
We are a private independent Company and formed through our own lived experience. The Founder and the directors have worked collaboratively in Government and Service delivery in Australian Public Service roles, Employment Providing including Recruitment, and Management Positions which make for an elite skill set.
We are also a registered Carers Gateway company. We are focused on dual support being mental health recovery through person centred oriented practices as well as Disability Support.
Currently I offer Psychosocial Recovery Coaching via TeleHealth sessions for the Cairns region.
We are taking referrals now for NDIS Disability Support for:

- hosting in house respite

We specialise in culturally appropriate Psycho-social recovery to Indigenous participants and also provide Support Coordination and Access to Community/Social & Rec supports.
We provide full support and transport to all appointments and social outings to CAIRNS - EDMONTON - GORDONVALE - YARRABAH & ALL SURROUNDING AREAS.
