Disability Matters Inc, is the Tablelands largest NDIS Provider, with over 20 years experience in the arena.
We provide Plan Management, Support Coordination, and Direct Support (including in home, community access, Supported Independent Living, and Short Term / Medium Term Accommodation). We provide services from Cardwell to Cape York, although the direct support activities are limited by reasonable driving range for our Tablelands based support workers.
Individual support for you - in home or in the community
Disability Matters provides individual support to clients either in their own home or in the community.
Whether it’s personal care, capacity building to develop independent living skills in and around the home or even help with household tasks, Disability Matters can help to provide personalised supports for you.
We can assist with social and community participation and the development of daily living skills. We also have staff that can provide employment and education support. Members of our Coordination team are also trained in behaviour management and can offer you strategies for managing behaviours.
Our staff have received training on the importance of focusing on client goals at each support shift and reporting on those goals. Including people with a disability in community life helps build more inclusive and diverse communities. It enriches community life for everyone and helps break down barriers and promote better understanding.

We have an amazing team of dedicated and experienced staff and we conduct recruitment of new staff on a regular basis. Many of our staff have completed their Certification III in Individual Support and we have also put many of our staff through high intensity workshops so they have the required skills to support individuals who need that extra level of care.
Housing and accommodation - Supported accommodation for you
For those times when everyone just needs a break, we provide 24-hour support in accommodation at our Cook St house in Atherton.
The house provides overnight care, 7 days per week. With up to four clients able to stay at a time, the house has built up a reputation of being a caring, supportive and enjoyable “home away from home” for people living with a disability.
Clients are encouraged and supported to participate in a variety of activities whilst at Cook Street, both indoors and outdoors. Some activities we offer at Cook Street include:

🟠 Fun days with BBQ’s and games
🟠 Outings into the community and local attractions including Tinaroo Dam, Lake Eacham, local parks and whatever other activities or events are being held
Younger clients at the home often feel they have gained some independence away from their families whilst their parents have peace of mind knowing that their child is being appropriately supported.
Our support at Cook Street is based on a 1 support worker to 2 clients (1:2) ratio, however, we have the ability to scale down to 1:4 or up to 1:1.
Supported independent living for you
Disability Matters manages supported independent living (SIL) houses where housemates are provided with 24/7 support. This is their home!
Supported independent living is a wonderful opportunity for people who wish to live their own life, have the company of a housemate/s and there is the comfort and reassurance that care and support is in place all day every day.
Our support workers provide assistance with banking, shopping, domestic chores and skill development if and when required. Assistance and support are provided to enable the residents to pursue their own particular interests, and become contributing members of their community. This support strengthens the client’s independence and gives peace of mind to their, often elderly, parent carers.
Disability Matters can source a home for those who are interested in pursuing this option. We will guide you through the NDIS pathway to gain the required SIL funding and if required also find you a compatible housemate.
If you are interested in SIL in the future, but not quite ready yet, we can place you on a ‘waiting list’ for when a vacancy becomes available in one of our existing houses.

Prior to opening Your Choice SC I was employed in a full time Support Coordination position in Townsville and became very driven by the importance and efficacy of independent support coordination and the transparency that independence in this role is paramount to client success.

Supported Independent Living (SIL) is an NDIS funded model of shared accommodation. At Care FNQ, our staff are there to help with supporting our participants with all area's of daily tasks in a independent or shared living environment. We focus on developing the skills of each individual to live as independently as possible. The support is provided to each person living in the shared arrangement based on their support needs. Support Coordination specific to individuals living with MND. Advise and support for individuals living with MND who cannot receive NDIS funding.
(MND) is a disease that often progresses quickly, therefore, having a great support network around the person affected with MND is seen as essential in achieving the best possible quality of life. Our MND Advisors work exclusively with persons diagnosed with MND and therefore have developed an extensive understanding of the complex needs of those living with this disease.
Additionally our advisors are highly educated in the various funding models available to support persons with MND and also are well connected with local providers who can deliver the supports that may be beneficial.
Our MND Advisors are also Support Coordinators under the NDIS, which means that persons with MND can receive both expert advice as well as specialist support in managing their plans and their providers.

My background is in child protection and behaviour support practice in NDIS. I have a Master’s in Social Work, Graduate Certificate in Human Services Management, and a Bachelor’s in Business – HRM.
I am originally from Papua New Guinea (PNG) and have been in cairns for the past 23 years.

We give you a solid understanding of the NDIS, legislation and framework paired with practical tools and templates to run your business. Tertiary education such as a Certificate in individual supports are a great start but just don’t include NDIS knowledge and practical tools to get you started. Our expert trainer has worked extensively for the NDIS, has written and audited NDIS plans for quality assurance and has trained Local Area Coordinators in Far North Queensland. We know both sides of the system and the complexities that come with it and are true believers in the power of knowledge. Tivanee is a social worker who holds registration with the Australian Association of Social Workers (AASW). She is passionate about taking therapy outside a clinical setting and reducing barriers to services.
Tivanee listens to her client’s need and preferences and provides counselling in a safe, non-judgmental space. Trained in Sandplay Therapy and Outdoor Therapy, her approaches allow opportunity for clients to explore feelings and experiences often inaccessible or difficult to express in words. She can offer practical techniques to help people improve overall health and wellbeing, cope and manage their day-to-day, and support their journey of healing and growth.

I mainly support people with degenerative neurological conditions such as MND and MS.
I only support people that is over 18yo.
