Transition Guide – Supporting Teens and Young Adults with Disability

Updated August 2026!

Written and collaborated by Sean Diehm & Jewel Skelly of Being Me Therapies, Rona Bartlett of All At Once Consulting, and Heidi of Cairns Disability Network.

This (Cairns version) guide is full of helpful information about support services and things you should know as a young adult.

You can read this guide on your own, or it might be more helpful to go through it with a caregiver, parent, caseworker, or someone else who knows you well.

If you’re unsure about anything, or if you need help finding information or filling out forms, don’t hesitate to ask your caseworker or provider for assistance.

As you transition to independence, it’s important to know what services and options are available to you both before and after you leave care. Even after leaving care, you should continue to receive support for things like continuing your education, finding a job, securing safe housing, or having someone to talk to when you need help.
