About Us:
Sacred Heart Care PTY LTD was established in 2023 with a vision to provide a high quality of in-home support and disability services to the Northern Beaches community. The team at Sacred Heart Care are experienced health care professionals with extensive knowledge and experience in Clinical Nursing and Disability/Lifestyle support.
Our Qualifications:
The team at Sacred Heart Care hold qualifications in the following: Bachelor of Psychological Science, Master of Clinical Nursing, Graduate Diploma of Mental Health Nursing, Diploma of Counselling (in progress), Accounting and Business Management. We are First Aid/CPR Certified, Blue and Yellow Card Certified, Full Drivers Licence and registered through ASIC.
Our Mission:
In order to help you feel supported within your own home, our caregivers will formulate an individualised care plan that will assist you with any specific health care needs. We will tailor a care package that suits your lifestyle and goals, whether you are plan managed or self managed via NDIS. Your health and wellbeing is our priority, we are passionate about assisting you to achieve a healthy mind and body.
At Sacred Heart Care, we believe that your home is a sacred space and our aim is to help you create an environment which not only brings you comfort, but more importantly a sense of independence. 24/7 support is available, our team will make every effort to make you feel valued and heard.
Services Provided:
Private Rental/SIL Accommodation available mid to late 2024, further accommodations available in 2025. Affordable and pet friendly. This is suited to NDIS Participants that are Plan Managed or Self Managed.
Gardening and household chores
Minor maintenance jobs
Assisting with meal planning, grocery shopping and cooking
Activity Based Transport - e.g., transport to appointments
Assistance with Personal Care
Access to Community, Social and Recreational Activities
Medication Administration - Oral, Subcutaneous and Intramuscular medications, we can administer, supervise and support you with this task
Psychosocial Support - Emotional/mental health support is very important to us and we believe that it is just as (if not more important) than our physical health. A holistic approach is highly regarded by Sacred Heart Care
Social Interaction/Companionship - A coffee, movie or walk by the beach, one-on-one or group based
Telehealth support - If you simply need someone to talk to, our team are available via telephone and/or video call (if you are not tech savvy, that's ok, we would be more than happy to assist you with this)
