We are experts in Support Coordination and Plan Management services, with a prominent focus on remote regions. We understand the importance of Aboriginal and Torres Strait Islander cultural practices, traditions, and protocols within the various communities. Xtremecare is committed to ensuring that our service is tailored to each individual and community’s needs.
We aim to build up the capacity of participants and communities to empower them to deliver NDIS services in their community, which will create long term sustainability in the future.
We provide our Plan Management service nationally, our Support Coordination service is delivered only in the Western Cape Region, Northern Peninsula Region, and Torres Strait Regions.
